They r just S M L XL could write company for better idea of the sizes

I hand wash mine in the sink and let it air dry. Seems to be fine.

Are you supposed to measure from the middle or the top of the knee cap? i'm either a l or xl, so it matters.

Always above the knee at least 4" above it. Its to measure the width around your leg so it won't be to tight and hurt.
