What is the difference between the 2 models?

I don’t know. But I’ve used mine for over a year in my car and it’s just perfect for lumbar support. It hold it shape. It holds its firmness.

How do you affix this to an office chair? The attachments that it comes with do not allow for the support to stay in a position.

I just sat mine where I wanted it and sat against it. It was never attached.

Does the shape of this work to keep pressure off the spine?

I wouldn't say so. You can try it and return it if not. I used this for about two months and I think it ended up making my lower back worse. I ditched it, adjusted my seat more upright, maxed out my lumbar and I'm better off without. If your pressure gets worse, seek out the most notable chiropractor you can! Good luck :)

What is the dimensions of the "B" style auto pillow?

I do not know the answer because I apparently purchased the "A" style auto pillow (much lower in height). This newer and larger version of the auto pillow looks to be more comfortable than my lower pillow. Usually the description of the item in Amazon's website gives the dimensions of its products.
