Why does the picture show a different set of cleats than what the description talks of?

The picture shows sh 51…they’re black. 56 are silver colored.

Are these multi-directional cleats?

These are standard SPD cleats, clip in front to back, you can un-clip left or right. They work well for me, I hope this helps.

does this item include the nut plate?

The nut plate that goes inside your shoe is supplied with Shimano shoes not with the cleats. Not sure about other brands of shoes.

Like the NBA guys on TNT say - Come on man!! The description says SH51. the photo of the package says SH56. which one is it? I want the black SH51.

I am not sure either, but I think it is sh-51. But more importantly the two are different the sh-56 is multi directional so it is easier to get out of, and the sh-51 is only one direction which is out ward away from the frame. Hope this helps.
