Is there a way to allow the hot water to get hotter than 38 degrees?

I do not have this valve but I have a similar one. On mine you push the small knob on the adjuster in.It is spring loaded. This lets you turn the temp knob further round increasing the water temp. This is a built in safety device to prevent accidental scalding

Is there a separate input for Hot water and cold water?

Yes it is like a T-bar - Hot water goes in the left input, cold water goes in the right input and the controlled version comes out the top (towards the shower head). Don't transpose the hot and cold inputs as the thermostat wont work as the hot water is low pressure (from your combi) and the cold water it higher mains pressure

No surprisingly there weren't any enclosed with the product. However you can download them from their website, it took me a while on their website to find the link but it is there if you search hard enough.
