It fits in the 1/4" chuck of the 3/8" end

Klein tools are supposed to have a lifetime warranty. How can i exchange it? The 5/16 nut driver part is wore out and doesn't work.

Contact Klein tool company directly. I believe they have a warranty department. They should be able to tell you their procedure for replacements.

How do I get to other screws?

The bits are in the long metal shaft. Pull hard on the metal shaft and it will pop out.
