I measured one of mine and it is just slightly under 12" square at the top. I hope this helps.

Are these made in the USA?

It said made in USA in the product details. When I received them there were no stickers on them saying anything different. They are extremely sturdy and well made, I purchased them about a year ago.

Do the basket come disarmed?

Not completely. The bottom is out and then you just open the basket up and lower the bottom. Two steps.
