Damaged adopter
We have 8 units of the adopter (specifically choosing Anchor Panasonic make, of which 4 in use. One unit was found damaged with the terminal separated from the body, screw(bolt) connecting the terminal to the body was damaged. Luckily these loose items did not come in contact with the other live sockets
