Do you need to wet them first, the same way you should with pour-over?

Technically no. If you are using it at the bottom of the portafilter I would recommend wetting it especially if you WDT before tampering. If you are using it on top of the puck (like a puck screen) wetting it is totally unnecessary.

How does this fit the bottom of a vst or ims basket?

It fit well on the top but would be a little bit big to fit on the bottom. Cuz the diameter at the bottom is a little smaller than the diameter at the top.
