Does it include a scoop in the tub?

There should be a scoop in each bottle.

What size scoop (ml) should you use to get the 25g serving size? Mine never came with a scoop.

Hmm, that's a bit annoying :S If you have an old protein bucket then check what the serving is on that one and you could use that scoop. If this is your first bucket then i would suggest using a one-use cup (or normal cup) and a kitchen food scale (if you have one). If you don't have those then i guess it should be 3-5 full-spoons of the protein powder. (i am not hot just now so i can not verify that. i can send another answer tonight testing that.

How grams of carbs per serving?

A 25g serving has 1.3g carbs of which there is .5g sugar.
