are these ball end or do these come in ball end versions?

As far as I know, all D'Addario "steel" string packs come with ball ends. To my knowledge, only the Nylon Classical strings come with/without ball ends.

how many strings in the pack

It's not a dumb question. Amazon is sending out single strings instead of complete sets in some cases. Happened to me 3 times when buying D'Addario electric "set"s.
