Does it come with an RJ45 Port?

This unit comes with one empty keystone port. You have to buy the keystone jack separate and snap it in.

I have a 5" hole - will this work with that floor hole?

These are designed to be used with a PVC concrete junction box (FLBC4500/FLBC4502 or similar / purchased separately). These are 4.5" ID and 5" OD. So if you have one of those PVC junction boxes, it will fit into a 5" hole in your floor. The top trim plate is 5.625" so it will cover the hole + most cutting imperfections.
